CHILD CUSTODY AGREEMENT
THIS CHILD CUSTODY AGREEMENT (“Agreement”) is made and entered into on [Date], BETWEEN:

AND
[Parent 2 Name], residing at [Parent 2 Address] (“Parent 2”).
WHEREAS

Parent 1 and Parent 2 are the parents or legal guardians of the following minor child(ren):

The parties wish to agree upon the custody, care, and support arrangements for the child(ren) listed above.
NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, the parties agree as follows:
1. DEFINITIONS
1.1. Physical Custody: The right and responsibility to provide a home for the child(ren) and to make day-to-day decisions regarding their welfare.
1.2. Legal Custody: The right and responsibility to make major decisions regarding the child(ren)’s health, education, and welfare.
2. CUSTODY ARRANGEMENTS
2.1. Legal Custody

5. CHILD SUPPORT

8. DISPUTE RESOLUTION
8.1. If disagreements arise regarding this Agreement, the parties shall first attempt to resolve the matter through discussion or mediation before seeking court intervention.
9. MODIFICATION
9.1. This Agreement may be modified only by a written document signed by both parents or by court order.
10. GOVERNING LAW
10.1. This Agreement shall be governed by and construed in accordance with the laws of [Governing Law State/Country].
11. ENTIRE AGREEMENT
11.1. This Agreement contains the entire understanding between the parties concerning the subject matter herein and supersedes all prior agreements or understandings, whether written or oral.
