CUSTOMER TESTIMONIAL RELEASE FORM
THIS AGREEMENT is made on [Date], BETWEEN:

WHEREAS
- The Customer has agreed to provide a testimonial regarding their experience with the Company's products or services.
- The Company wishes to use the Customer’s testimonial for promotional, advertising, and marketing purposes.
NOW, THEREFORE, in consideration of the mutual promises herein, the parties agree as follows:
1. Grant of Rights
1.1 The Customer hereby grants the Company the irrevocable, worldwide, royalty-free right to use, reproduce, display, publish, and distribute the Customer’s testimonial, including any accompanying images, video, or audio provided by the Customer, in any media or format, whether now known or hereafter developed.
1.2 The Company may edit, adapt, or modify the testimonial for length or clarity, provided that the original meaning is not materially changed.
2. Use of Name and Likeness
2.1 The Customer authorizes the Company to use the Customer’s name, likeness, and other identifying information, as provided by the Customer, in connection with the testimonial.
2.2 The Customer may request the use of initials or anonymous attribution by notifying the Company in writing at [Company Contact Email/Address].
3. No Compensation
3.1 The Customer acknowledges that they will not receive any monetary payment or other compensation for the use of the testimonial, except for any consideration expressly stated here: [Specify any consideration, if applicable; otherwise, write "None"].
4. Representations and Warranties
4.1 The Customer represents that the testimonial reflects their honest opinion and experience with the Company’s products or services.
4.2 The Customer affirms that the testimonial content is original, does not infringe upon any third-party rights, and that they have the authority to grant the rights described in this agreement.
5. Release and Waiver
5.1 The Customer releases and discharges the Company from any claims, demands, or causes of action that may arise from the use of the testimonial as permitted by this agreement, including claims for defamation, invasion of privacy, or infringement of rights of publicity.
6. Term and Termination
6.1 This release is effective as of the date signed below and shall remain in effect indefinitely unless revoked in writing by the Customer. Revocation will not affect uses of the testimonial made prior to receipt of such revocation.
7. Governing Law
7.1 This agreement shall be governed by and construed in accordance with the laws of [Governing Law State/Country].
8. Entire Agreement
